We welcome new patients to enrol with us and look forward to helping you with your health needs.
Enrolling with Waimairi Road Family Medical Centre is easy.
Simply print and fill out the enrolment form, and health questionnaire found on our website for each person, then sign and date it. Parents should sign on behalf of their children who are under 16 years old. You can then drop the enrolment form in to us, along with your photo identification, which could be your passport or driving licence.
If you are not a New Zealand citizen, you need to bring your passport which shows your resident visa and/or work visa as well as your date of entry into New Zealand to establish your eligibility for enrolment.

If you are not eligible to enrol you can still come to Waimairi Road Family Medical Centre for consultations as a casual patient. Casual fees will apply.
Please note the Chinese version of the enrolment form on our website is only provided for your guidance, and we still require the English version of the form to be completed.
