[vc_row][vc_column][vc_column_text css=””]Currently we have limited spaces for patients who want to transfer from another practice in Christchurch. We might be able to consider new patients whose family members are already enrolled at our practice. You are welcome to call 3585535 and check if we are able to take enrolments or add your name on the waitlist. <\/span>If we would have the capacity to take your enrolment, you can<\/span> <\/span> print and fill out the enrolment form<\/a><\/span>, and health questionnaire<\/a> <\/span><\/span><\/span>found on our website for each person, then sign and date it.\u00a0 Parents should sign on behalf of their children who are under 16 years old. You can then\u00a0drop the enrolment form in to us, along with your photo identification, which could be your passport or driving licence.<\/span><\/p>\n

If you are\u00a0not a New Zealand citizen, you need to bring your passport which shows your resident visa\u00a0and\/or work visa as well as your date of entry into New Zealand to establish your\u00a0eligibility for enrolment.<\/span><\/p>\n
